Modelling the molecular basis of cardiac repolarization.

چکیده
AIMS To study the properties of ion-channel gating (I(Ks), the slow delayed rectifier K+ channel) that underlie the channel's participation in rate-dependent repolarization of the cardiac action potential (AP). METHODS Computational biology approach was used to simulate the channel gating and the AP of a mammalian ventricular myocyte. RESULTS At fast rate, channels accumulate at closed state near the open state, from which they can rapidly open to generate large repolarizing current late during the AP, effectively shortening its duration. CONCLUSION I(Ks) builds an 'available reserve' of channels that can open 'on-demand' to repolarize the AP and shorten its duration at fast rate ('rate-adaptation'). This property also makes I(Ks) effective in providing repolarization reserve when other repolarizing currents are compromised by disease or drugs.

Modelling and imaging cardiac repolarization abnormalities.
Repolarization abnormalities, including those induced by the congenital or acquired long QT (LQT) syndrome, provide a substrate for life-threatening cardiac arrhythmias. In this article, we use computational biology to link HERG mutations mechanistically to the resulting abnormalities of the whole-cell action potential. We study how the kinetic properties of I(Ks) (the slow delayed rectifier) t...
متن کاملPathophysiological basis and clinical application of T-wave alternans.
We review the contemporary understanding of the pathophysiology of repolarization alternans and present a perspective on the use of T-wave alternans (TWA) as a risk stratification marker of malignant ventricular arrhythmias. Several studies have demonstrated a high correlation of susceptibility to ventricular arrhythmias and sudden cardiac death with the existence of TWA. We describe a number o...
متن کاملHeart repolarization changes after anthracycline therapy in the children with cancer
Background Anthracyclines are important components of many chemotherapeutic protocols. The present study aimed to evaluate the repolarization changes in electrocardiography (ECG) which may predict drug induced arrhythmia. Materials and Methods In this cross-sectional study, the recorded ECGs were assessed for QT dispersion (QTd), QT corrected dispersion (QTcd), T peak to Tend dispersion (TPe...
متن کاملSimulations of propagated mouse ventricular action potentials: effects of molecular heterogeneity.
The molecular heterogeneity of repolarizing currents produces significant spatial heterogeneity and/or dispersion of repolarization in many mammalian cardiac tissues. Transgenic mice are prominent experimental models for the study of the molecular basis of repolarization and arrhythmias. However, it is debated whether the small mouse heart can sustain physiologically relevant heterogeneity of r...
متن کاملCardiac repolarization analysis using the surface electrocardiogram.
Sudden cardiac death (SCD) is a challenging health problem in the western world. Analysis of cardiac repolarization from the electrocardiogram (ECG) provides valuable information for stratifying patients according to their risk of suffering from arrhythmic events that could end in SCD, as well as for assessing efficacy of antiarrhythmic therapies. In this paper, we start by exploring the cellul...
